Case Managers liaise with various stakeholders including providers, multidisciplinary teams, and families. They conduct regular safe and well checks (every 4 – 6 weeks), offering continuous support and advocacy for the young people in Tier 4 care.

They work closely with the Quality Team and Assurance Team at Toucan, and ensure legal documentation is in place and up to date where needed. They also closely coordinate with community teams and social care services to help with discharge planning.

Their goal is to ensure that each young person receives the most suitable care and treatment at the right time, minimising restrictions, and optimising outcomes. Case Managers extend support not only to the individuals themselves but also to their families and carers, recognising the importance of holistic well-being.
